The National Committee for Quality Assurance (NCQA) reviewed the submission, and the practice has received NCQA Patient-Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long‐term, participative relationships.

About Miguel Faña, Jr., MD
Dr. Faña is board certified in internal medicine and has been practicing medicine since 1995. He completed internships and a residency in internal medicine at the renowned New York Hospital Medical Center of Queens in Flushing, New York, where he was appointed Chief Medical Resident. While at New York Hospital Medical Center Dr. Faña also served as Emergency Medicine Attending Physician and Junior Teaching Attending Physician. He received his Doctor of Medicine degree in 1995 from Pontificia Universidad Catolica Madre y Maestra. Dr. Faña is certified by the American Board of Internal Medicine and is a Fellow of the American College of Physicians.
About Rafael Velasquez, MD
Rafael Velasquez, MD, is board certified in family medicine. He attended residency in family medicine at the University of Alabama in Birmingham, Alabama and completed residency at Kansas University School of Medicine where he served as Chief Resident in Family Medicine. Dr. Velasquez was born in Lima, Peru, and graduated from Federico Villareal School of Medicine. He is a member of the American Academy of Family Physicians (AAFP), and the American Medical Association.
