The National Committee for Quality Assurance (NCQA) reviewed the submission, and the practice has received NCQA Patient-Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long‐term, participative relationships.
Earning PCMH recognition is a distinction in which a practice can take pride. The NCQA Patient Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduced cost. Medical Homes foster ongoing partnership between patients and their personal clinicians, instead of approaching care as a sum of episodic office visits. About Jatin Sheth, MD
Jatin Sheth, MD, is board certified in family medicine and has been affiliated with Access Health Care since 2003. Dr. Sheth’s medical background and years of experience make him a valuable member of the Access clinical team. In fact, everyone in his family is a physician and he was determined to maintain the family tradition.
Dr. Sheth received his Bachelor of Medicine and Bachelor of Surgery from B.J. Medical College, Ahmedabad, India in 1991. From 1991 to 1996 he served as Chief Medical Officer at Karnavati Hospital in India. Dr. Sheth relocated to the United States in 2000 and began his family practice residency in South Nassau Hospital in Oceanside, New York, where he was named Chief Resident.
